Summertime Check-Up

Would you like to schedule your child to come in for a checkup?

If you are a new patient and have insurance:
1. Fill out all required forms and we will take care of the rest

If you are a new patient and do not have insurance:
1. Fill out all required forms and we will sit down and discuss with you the prices and treatments that will need to be done.

If you are an existing patient and have insurance:
1. Drop your child off and we will take care of the rest. After we complete the treatment we will call you to pick up your child

If you are an existing patient and do not have insurance:
1. Drop your child off and we will take xrays of your child's current case. We will then discuss what treatments will be needed, and then discuss the pricing with you.
